Introduction

Adventist Health/Medical Group is pleased to offer patients access to their electronic medical record through the use of MyChart.

As you sign up for MyChart, it is important for you to know what to expect from this new service, how we will handle the exchange of information communicated over the Internet, and how your health care experience will change as a result of using this program.

This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. Please be aware that these terms and conditions may change without notice. We encourage you to review them from time to time to familiarize yourself with any updates. We reserve the right to revoke MyChart account access at any time for any reason. You will be notified in writing if such a revocation has taken place.

MyChart Services Rendered

The MyChart, MyHealth offers a wide availability of services which includes:

  • The ability to request or directly schedule clinic appoinments without having to wait on the phone,
  • The ability to view and print your medication list (complete with instructions on how to take them), immunization and allergic reaction records, as well as upcoming preventative care reminders,
  • The ability to view your labwork and other test results more quickly and securely than through postal mail delivery,
  • The ability to request prescription renewals from your clinic electronically when your allotted refills have run out at your pharmacy,
  • Access to trusted health information resources to assist you in researching medical conditions, treatments, procedures, medications and more,
  • The ability to track your health progression by graphing your lab and test results over time,
  • The ability to pay your clinic bill securely online with a credit card,
  • The ability to view your family's medical records, make appointments and request prescription renewals on their behalf (proxy access),
  • Electronic health histories and questionaires that will help avoid your having to fill out paperwork at your clinic.

Changes to Your Health Care Experience

As you sign up for MyChart, a few changes in the way your health care is managed will take effect immediately. We want you to be aware of these items so that that they do not come as a suprise to you. Your use of the MyChart website serves as permission to receive certain items electronically, rather than by phone or postal mail.

  • Test Results: Lab and certain test results will be distributed to you electronically through the "Test Results" section of MyChart (found under "Health"). You will no longer receive them through the mail or by phone. In certain cases, your health care provider may wish to discuss your test results with you in person, and therefore will not release them to you via MyChart. In such cases, your provider will request that you return to the clinic for a follow-up visit where you can review your lab results together.
  • Announcements, Reminders and Other Mailings: Clinic-generated updates, announcements and preventative care reminders (such as a reminder that it's time to get your tetanus shot) will now be delivered electronically to your MyChart inbox, rather than by postal mail.

MyChart also offers users the unique convenience of communicating certain clinic processes via the Internet rather than by phone. As you take advantage of these services, certain considerations are important.

  • Online Scheduling: There are two new ways in which you may make an appointment with your clinic through MyChart. You may either electronically request an appointment which will be made by your clinic staff within the parameters of your preferred times and dates, or you may actually schedule an appointment with the clinic yourself. In doing either, please be sure to record the date and time of your appointment and to arrive 15 minutes early. You may cancel an appointment that you have requested electronically on MyChart up to two (2) business days in advance of your scheduled appointment. If you need to cancel within a shorter time frame, please call the clinic to cancel. Three (3) "no-show" appointments that were scheduled via MyChart will result in the permanent loss of your online scheduling privileges.
  • Prescription Renewal Requests: Another convenience offered is the opportunity to request prescription renewals from your clinic via MyChart, rather than by contacting the clinic by phone. Please note that renewals are not the same thing as refills. Refills are indicated as the amount of times your provider has allowed your pharmacy to refill your prescription. You should contact your pharmacy itself for refills with no need of clinic intervention. Renewals, however, are the request made to your provider to communicate with your pharmacy again for the same prescription when your allotted refills have run out. It requires your provider to contact the pharmacy and notify them that you may once again have an allotted amount of refills on a specific medication. Renewals are made at the discretion of your provider and may sometimes be denied, or sometimes may require a follow-up visit before your provider will grant your request.

Your Online Medical Record

Access: MyChart does not display all of the information found in your medical record. If you would like to request a complete transcript of your medical record in hard copy, please contact your clinic and they will have you fill out the necessary Release of Information forms.

Accuracy: Adventist Health/Medical Group strives to retain our medical records as accurately as possible. Should you find something to be in error as you review your medical record through MyChart, please take note of it and bring it to your provider's attention at your next clinic visit.

Restrictions: Please be aware that certain topics (such as HIV and AIDS-related information) will never be released via MyChart due to federal laws. Also, mental health and substance abuse-related topics will be generalized under non-specific umbrella categories to protect your privacy.

Response to Electronic Communication

Response Time: Adventist Health/Medical Group will make its best effort to provide a timely response to electronic inquiries. In some cases, the clinic staff that needs to respond to an electronic communication may not be immediately available. Patients should allow at least three (3) business days for a response. Accordingly, emergency situations requiring immediate attention should NOT be submitted electronically.

Patient Responsibility: With respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we will be unable to provide accurate and reliable services.

Health Library Reference

As a service to our MyChart patients, Adventist Health/Medical Group offers a link to the MedlinePlus. You can access this helpful resource through the "Search Medical Library" under Resources tab on the homepage. Though we encourage our patients to use this tool, Adventist Health/Medical Group has not verified the accuracy of the information in this reference. Patients should not rely on any of the information found on the Health Library for purposes of treatment or diagnosis, nor should this reference serve as a replacement for the advice of your personal physician.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by Adventist Health/Medical Group in any other medium. Adventist Health/Medical Group is committed to protecting the confidentiality of your medical information. We limit Adventist Health/Medical Group employees’ access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient accesses MyChart. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Adventist Health/Medical Group by viewing the digital certificate. This certificate verifies the connection between the Adventist Health/Medical Group server's public key and the server's identification.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet. As a MyChart user, your role in maintaining the security of your medical information is:

  • Changing your password on a regular basis, and
  • Keeping your login ID and password confidential.
